- One (1) Comprehensive Exam
- One (1) Full Mouth Series (FMX)
- Unlimited Emergency Exams with PA as needed
- 20% OFF any dental procedure needed

CHILD PLAN (0–13 YEARS)

Annual Membership Fee: $325
Includes:
- Two (2) Child Cleanings per year
- One (1) Periodic Exam
- Bitewing X-rays as clinically indicated
- Fluoride treatment (when recommended)
- Unlimited Emergency Exams
- 20% OFF any dental procedure needed

- Membership valid for 12 months from enrollment date
- Fees are non-refundable
- Benefits do not roll over
- Discounts apply to in-office dental procedures only
- Discounts cannot be combined with insurance or other offers
- External lab fees may be excluded
- Missed appointments subject to standard office policy
- Children are defined as 0–13 years old; 14+ are considered adults
